Physician Assistant Salaries: Top 8 Highest Paying Specialties for 2023

So, you’re interested in being a Physician Associate, but don’t know what specialty to pursue or maybe you’re about to graduate and want to optimize your earning potential? Maybe you're in one of the lower paying specialities and want to make a change? Well, you’ve come to the right place!

We are taking a look at the multitude of specialties you can choose from as a PA. Most other advanced practice professionals cannot move from one specialty to another, but PA’s are able to zig/zag to suit their fancy and their paycheck.

  • Anesthesia

  • Cardiology

  • Dermatology

  • Emergency medicine

  • ENT/Otolaryngology

  • Family medicine

  • General practice

  • Internal medicine

  • Neurology

  • Obstetrics and gynecology (OB/GYN)

  • Radiology

  • Pediatrics

There is a laundry list of potential specialties AND subspecialties (see above), but some are paid better than others, and may also bring better work/life balance. We aren’t advocating to ditch your interests over pursuit of coin, but rather trying to educate you on where you will see the most career growth over the next ten years. So, let’s dive in!



Physician Assistant: Cardiovascular/Cardiothoracic Surgery

  • Average annual salary: $147,200

  • Job outlook (2021-2031): 28%

Side note: I've heard that this is also one of the most demanding in terms of work/life balance!


Physician Assistant: Dermatology

  • Average annual salary: $146,000

  • Job outlook (2021-2031): 28%

Side Note: I've heard that this is one of the most challenging to break into


Physician Assistant: Emergency Medicine

  • Average annual salary: $129,146

  • Job outlook (2021-2031): 28%

Physician Assistant: Surgical Subspecialties (ex. Orthopedics)

  • Average annual salary: $127,775

  • Job outlook (2021-2031): 28%

Side note: I've heard that Orthopedics/Sports Med is one of the BEST specialties when it comes to both pay and work/life balance.


Physician Assistant: Occupational Medicine

  • Average annual salary: $125,600

  • Job outlook (2021-2031): 28%

Physician Assistant: Critical Care

  • Average annual salary: $124,886

  • Job outlook (2021-2031): 28%

Physician Assistant: Neurosurgery

  • Average annual salary: $124,000

  • Job outlook (2021-2031): 28%

Physician Assistant: Urgent Care

  • Average annual salary: $123,517

  • Job outlook (2021-2031): 28%

If you’re passionate about one of those specialties, you’re in luck! If not, don’t fret. There is still an average of a 28% growth rate between all specialties for PAs over the next ten years! Supply and demand.
